Chicagoland Infectious Disease is seeking a compassionate Nurse Practitioner, Family Nurse Practitioner, or Advanced Practice Registered Nurse to deliver in-person care in skilled nursing and post-acute settings throughout Indiana. This part-time, Monday through Friday opportunity offers no nights or weekends, collaborative physician support, flexible scheduling, and a rewarding path to grow in infectious disease care without requiring prior specialty experience.

What You Will Do:

  • You will conduct comprehensive assessments and diagnose, treat, and monitor acute and chronic conditions for patients in skilled nursing facilities.
  • You will evaluate infectious conditions, review laboratory and diagnostic findings, and develop individualized treatment plans.
  • You will prescribe and manage medications within your scope of practice, adjusting care plans as patient needs and clinical outcomes evolve.
  • You will collaborate closely with physicians, nurses, facility teams, patients, and families to coordinate high-quality care.
  • You will provide education on treatment planning, infection control, prevention practices, and medication management.
  • You will complete accurate, timely clinical documentation and help advance strategies that reduce avoidable hospitalizations.

Required Qualifications:

  • You hold an active Nurse Practitioner or APRN license in Indiana and maintain board certification through the American Nurses Credentialing Center or another recognized certifying body.
  • You have completed a Master of Science in Nursing or an equivalent advanced practice nursing degree.
  • You maintain current DEA registration or are eligible to obtain a DEA license, along with current BLS certification.
  • You bring strong assessment, diagnosis, clinical decision-making, and patient communication skills.
  • You are comfortable practicing independently while collaborating effectively with physicians and interdisciplinary care teams.

Preferred Experience:

  • Experience in primary care, gerontology, internal medicine, urgent care, acute care, psychiatry, wound care, hospice, hospital medicine, or outpatient care will translate well to this role.
  • Previous work in skilled nursing, long-term care, post-acute care, telehealth, or medication management is valued but not required.

Compensation and Benefits:

This position pays $50 per hour. Eligible team members may access health, dental, vision, life, short-term disability, and long-term disability insurance, along with a 401(k), health savings account, paid time off, and flexible scheduling.
