PRN Physical Therapist (PT) – In-House Rehab Aventura at Shiloh Springs – Trotwood, OH Looking for a flexible PRN opportunity with a team that values great therapy and great teamwork? Aventura at Shiloh Springs is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team! This is a great opportunity for a PT who enjoys working with the skilled nursing and long-term care population and wants the flexibility of a PRN schedule while making a meaningful difference in residents' lives. What We Offer Flexible PRN scheduling Competitive PRN rates In-house rehab environment Supportive and collaborative therapy team Opportunity to work with a diverse SNF/LTC population Net Health for therapy documentation PointClickCare (PCC) for facility documentation What You'll Do Evaluate residents and develop individualized physical therapy plans of care Provide skilled PT services focused on mobility, strength, balance, safety, and functional independence Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team Provide resident and family education as appropriate Complete timely and accurate documentation Support safe discharge planning and continuity of care What We're Looking For Graduate of an accredited Physical Therapy program Current Ohio Physical Therapist license in good standing SNF/LTC experience preferred Experience with Net Health and PointClickCare (PCC) preferred Strong communication and teamwork skills Dependable, compassionate, and resident-focused Ready to Join Us?
