Responsibilities
- Provide direct patient care under the supervision of a registered nurse in a long-term care setting.
- Administer prescribed medications and document administration accurately in patient records.
- Monitor patient status, recognize changes, and communicate updates to the supervising RN; participate in care planning.
- Assist with activities of daily living (ADLs) and comfort measures to support resident well-being.
- Maintain accurate clinical documentation and ensure adherence to infection control and safety standards.
- Collaborate with the interdisciplinary team, respond to call lights promptly, and support patient safety and quality of care.

Requirements
- Active Licensed Practical Nurse license in good standing.
- Ability to work in a long-term care setting and fulfill a 12.5-hour shift schedule.
- Graduation from an accredited practical nursing program.
- LTC experience preferred.
- Strong communication and teamwork skills.
- Willingness to participate in a travel assignment and follow facility policies and procedures.
