Case Management Lead

JOB SUMMARY: To ensure high-quality care coordination and support for the Tepeyac Community Health Center patient population, the Case Management Lead provides day-to-day operational oversight, mentorship, and support for Case Managers. This role promotes effective care coordination, integrated care delivery, quality outcomes, and adherence to organizational standards while serving as a subject matter expert and resource for complex patient cases.
The Case Management Lead serves as the first-line clinical and operational resource for Case Managers regarding workflow coordination, patient care barriers, community resources, documentation requirements, and staff development. This position provides real-time coaching, training, and performance support, assists with workload balancing and coverage coordination, and escalates concerns as needed. The Case Management Lead partners closely with leadership to ensure consistent implementation of case management workflows, quality initiatives, and patient-centered care practices. This role also develops and maintains collaborative relationships with community organizations, referral partners, and other external stakeholders to strengthen resource accessibility, care coordination, and support services for patients and families.

PAY & BENEFITS: Tepeyac offers a full compensation package to all full-time employees. Benefits include medical, dental and vision insurance, a flexible spending account, a 401k, disability insurance paid by Tepeyac, eight paid holidays, four weeks of vacation and 6 days of sick leave each year. The hourly pay rate for this position is $32.22/hour.
MINIMUM QUALIFICATIONS:
Minimum Education
- Bachelor's degree in Social Work, Human Services, Public Health, Psychology, Behavioral Health, Nursing, or related field required.
- Master's degree in a related field preferred.
- Relevant case management certification preferred (CCM, ACM, or equivalent).
Minimum Experience
- Minimum of three (3) years of case management experience in a community health center, healthcare, behavioral health, social services, or integrated care setting.
- Experience working with complex and high-risk patient populations required.
- Prior experience serving in a leadership, preceptor, trainer, or lead role strongly preferred.
- Experience utilizing an electronic health record (EHR/EMR) system preferred.
- Experience with community resource navigation and care coordination strongly preferred.
Language Requirements
- Oral and written fluency in English required.
- Oral fluency in Spanish strongly required.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Team Leadership & Operations
- Provide day-to-day leadership, guidance, and support to Case Managers, serving as the primary resource for workflow questions, care coordination, and complex patient needs.
- Assist with workload balancing, caseload distribution, scheduling coordination, and coverage planning to ensure continuity of services.
- Mentor, train, and onboard new staff while promoting professional development and career advancement.
- Facilitate team huddles, case reviews, and departmental meetings; communicate operational updates and organizational priorities.
- Serve as a liaison between Case Managers, leadership, providers, and community partners.
Clinical Case Management & Care Coordination
- Participate as a key member of Tepeyac's integrated care model and collaborate with internal and external partners to coordinate patient care and address social determinants of health.
- Support Case Managers in managing complex cases by providing consultation, problem-solving assistance, and guidance regarding community resources, benefits, and care planning.
- Ensure patients receive comprehensive case management services, including assessment, individualized care planning, referral coordination, resource navigation, and retention in care.
- Promote trauma-informed, strength-based, patient-centered care practices that support self-management, patient engagement, and health outcomes.
- Maintain a caseload to support departmental operations and patient needs.
Ryan White Program Responsibilities
- Support Ryan White Case Management activities and ensure compliance with program requirements.
- Provide oversight and consultation regarding care plans, medication adherence, retention in HIV care, and service coordination for patients living with HIV/AIDS.
- Ensure required Ryan White documentation, reporting, and CareWare data entry are completed accurately and timely in accordance with grant requirements.
- Monitor program-related quality indicators and support continuous quality improvement efforts associated with Ryan White services.
Quality, Compliance & Documentation
- Monitor documentation, workflows, and program requirements to ensure compliance with organizational, regulatory, grant, and payer standards.
- Support quality improvement initiatives, audits, reporting activities, and performance metric monitoring.
- Ensure accurate and timely documentation within the electronic health record and related reporting systems.
- Promote compliance with HIPAA, patient confidentiality standards, and organizational policies and procedures.
Community Partnerships & Program Development
- Develop and maintain relationships with community organizations, referral partners, and government agencies to improve patient access to services.
- Identify opportunities to strengthen referral pathways, expand community resources, and improve care coordination processes.
- Participate in quality committees, program development initiatives, and departmental planning activities as assigned.
- Maintains a comprehensive understanding and working knowledge of all case management programs and services, providing support, guidance, and cross-coverage as needed to ensure continuity of care and program success.
Other Duties
- Attend required meetings, trainings, supervision sessions, and leadership development activities.
- Demonstrate flexibility, professionalism, and accountability while supporting departmental and organizational goals.
