Reports to: HIV Counseling & Testing Coordinator
Department: Prevention / ACCESS
Program: Outreach & Expansion
Annual Salary: $44,100 - $60,800

Major Function The Community Health Worker provides emotional, educational, and navigational support for clients experiencing barriers related to social determinants of health. This position works to engage, inform, support, and empower community members eligible for health center services to improve healthcare access, retention, and overall wellness.

Essential Functions

Outreach and Health-Systems Navigation:

Conduct community outreach and case finding to re-engage out-of-care individuals and bring those lost to follow-up into care services.
Maintain regular external communication, conducting routine check-ins, encouragement calls, and appointment reminders.
Accompany clients to medical, behavioral health, substance use treatment, and social service visits for affiliation support.
Assist clients with scheduling healthcare appointments and coordinate transportation resources to ensure attendance.
Collaborate with healthcare providers to plan and support Chronic Care Management.

Health Education and Care Coordination:

Provide health literacy support, treatment adherence education, and emotional encouragement to clients.
Guide clients on necessary documentation and steps needed to access social support programs, housing applications, and health insurance.
Walk clients through initial appointments for medical care or community services to ensure clear understanding of next steps.
Promote effective resource utilization across multidisciplinary care teams and maintain active tracking of patient referrals.
Represent and promote program services within the organization and to partner community organizations.
Attend mandatory agency meetings and ongoing professional development training sessions.

Documentation, Compliance, and Quality Assurance:

Maintain clear, timely, legible, and compliant chart documentation in accordance with agency standards.
Assist with the collection of client outcome data and participate in quality assurance chart reviews as requested.
Utilize health information systems and data tools to support productivity and continuous quality improvement.

Safety:

Follow standard precautions aligned with Centers for Disease Control and Prevention guidelines.
Maintain working knowledge and active execution of responsibilities under the Emergency Code System and Continuity of Operations Plan.

Additional Responsibilities

Maintain flexibility to adapt to changing organizational staffing needs and priorities.
Perform other related duties and special projects as assigned by program leadership.

Requirements

Education and Experience:

High school diploma or equivalent required; Associate or Bachelor's degree preferred.
Minimum of 1 year of experience conducting outreach and working with at-risk populations.

Knowledge, Skills, and Abilities:

Working knowledge of social determinants of health and regional community resources/support programs.
Computer literacy, including proficiency with Microsoft Word and Excel.
Bilingual proficiency (English/Spanish or English/Haitian Creole) preferred.
Strong interpersonal, organizational, and problem-solving skills with the ability to engage diverse and multicultural populations.
Self-motivated and detail-oriented with the flexibility to travel locally and work adaptable hours.

Physical and Work Environment:

Requires manual dexterity to operate computers, phones, and standard office machinery.
Physical ability to kneel, bend, and perform light lifting.
Clear verbal and written English communication skills with normal hearing and visual acuity.
Standard office and field-based community health environment.
