As a Licensed Practical Nurse (LPN) , you’ll provide compassionate, individualized care to adults with intellectual and developmental disabilities (I/DD) in home and community-based settings. You’ll work alongside a dedicated team that values empathy, respect, and empowerment — all while enjoying a more balanced, fulfilling pace. What You’ll Do Provide exceptional nursing care and oversight by implementing health care plans, charting vital signs, and administering medications as prescribed by physicians. Support individuals in their daily routines, offering personal care and health guidance as outlined in each Individual Support Plan (ISP). Build long-term, trusting relationships with the individuals you support and their families. Collaborate with other healthcare professionals, guardians, and team members to ensure holistic, person-centered care. Requirements and Responsibilities What You’ll Bring A current, active Connecticut LPN license. A valid Connecticut driver’s license with a clean driving record. Must successfully complete all required federal, state, and regulatory background checks as a condition of employment, including criminal history, driving record, and a pre-employment physical examination, in accordance with Connecticut and federal law. CPR & First Aid certification (or willingness to obtain — training provided). Compassion, patience, and a commitment to person-centered care. The ability to lift and support individuals with limited mobility when needed. (Preferred) Experience working with the Department of Developmental Services (DDS) nursing policies and procedures — training provided if needed.
