Amber Court of Elizabeth is seeking an experienced, compassionate, and highly organized Full-Time Nurse to support the health, safety, and overall well-being of residents within our assisted living community. Prior nursing experience in Assisted Living, Long-Term Care, Skilled Nursing, or a comparable senior-care setting is required for consideration. The Full-Time Nurse plays an integral role in ensuring residents receive appropriate clinical oversight while maintaining the dignity, independence, choice, and quality of life that are central to assisted living. This position requires more than strong clinical skills. We are looking for a nurse who understands the senior-living environment—someone who recognizes subtle changes in resident condition, communicates effectively with families and healthcare providers, maintains strong documentation, supports frontline staff, and takes ownership of concerns through resolution. The ideal candidate understands that successful assisted living nursing is often about recognizing a problem before it becomes an emergency. Essential Duties and Responsibilities Resident Assessment & Clinical Monitoring Conduct and/or participate in resident assessments and reassessments in accordance with New Jersey assisted living requirements, professional scope of practice, and community policy. Monitor residents for changes in physical, cognitive, behavioral, and functional condition. Recognize significant changes from a resident's baseline and ensure appropriate assessment, intervention, escalation, notification, documentation, and follow-up. Identify potential resident health and safety risks, including falls, changes in mobility, skin integrity concerns, nutritional or hydration concerns, medication-related concerns, and cognitive or behavioral changes. Monitor residents following hospitalization, rehabilitation stays, emergency department visits, falls, illness, or other significant changes in condition. Communicate clinical concerns promptly to the appropriate physician, healthcare provider, responsible party, and community leadership. Care Planning & Coordination Participate in the development, implementation, review, and revision of individualized resident service and care plans. Ensure identified resident needs and interventions are accurately reflected in the resident's plan of care. Coordinate care with physicians, advanced practice providers, pharmacies, hospitals, rehabilitation facilities, home health providers, hospice providers, and other members of the resident's healthcare team. Participate in resident care conferences as appropriate. Assist with resident admissions, transitions of care, and returns from hospitals or rehabilitation facilities. Ensure new and revised physician orders are appropriately communicated and implemented. Follow outstanding clinical matters through resolution rather than relying solely on initial notification or referral. Support residents' ability to safely age in place whenever their needs can appropriately be met within the assisted living setting. Medication Management Support safe medication management practices in accordance with New Jersey requirements, physician orders, professional scope of practice, and community policy. Review medication orders and medication-related documentation for accuracy and completeness as assigned. Identify and promptly address medication discrepancies. Communicate medication changes with appropriate staff, healthcare providers, pharmacies, residents, and responsible parties as necessary. Monitor and report potential adverse reactions, medication-related concerns, and changes in resident condition. Participate in medication audits and quality reviews. Provide clinical guidance and support to authorized staff involved in medication-related services within applicable scope and regulatory requirements. Falls, Incidents & Changes in Condition Respond appropriately to resident falls, injuries, acute changes in condition, and other resident emergencies. Perform nursing assessments within scope and determine when additional clinical intervention or emergency medical evaluation is necessary. Ensure required notifications and documentation are completed timely. Participate in post-fall and incident reviews to identify contributing factors and appropriate preventative interventions. Ensure changes in condition are appropriately communicated, documented, and incorporated into the resident's care plan when indicated. Promote a culture in which frontline employees promptly report near falls, behavioral changes, functional decline, appetite changes, confusion, weakness, and other deviations from a resident's baseline. Documentation & Regulatory Compliance Maintain accurate, objective, timely, and complete clinical documentation. Ensure resident clinical records are maintained in accordance with applicable New Jersey requirements, professional standards, HIPAA, and community policy. Review assessments, physician orders, progress notes, incident documentation, care plans, and other clinical records for completeness and accuracy as assigned. Assist with clinical record audits and implementation of corrective actions when deficiencies are identified. Support compliance with applicable New Jersey Department of Health requirements governing assisted living residences. Participate in regulatory surveys, quality-improvement initiatives, internal audits, and plans of correction as requested. Maintain required nursing credentials, education, and competencies. Immediately report suspected abuse, neglect, exploitation, mistreatment, or other reportable concerns in accordance with applicable requirements and community policy. Infection Prevention & Resident Safety Support the community's infection prevention and control program. Monitor and reinforce appropriate infection-control practices among staff. Assist with identification and management of communicable illness and infection concerns. Reinforce proper hand hygiene, PPE use, standard precautions, and other infection-prevention measures. Participate in emergency preparedness, evacuation, and disaster-response activities as required. Identify environmental or resident-care conditions that may create a safety risk and ensure appropriate corrective action. Staff Support & Clinical Education Serve as an accessible clinical resource to resident-care employees. Provide coaching and education regarding resident care, fall prevention, infection prevention, changes in condition, documentation, medication-related procedures, and other clinical matters. Reinforce the importance of immediately reporting changes in resident condition. Assist with staff competency validation and identification of additional training needs as assigned. Model professional, respectful, and resident-centered interactions. Collaborate effectively with Nursing, Resident Care, Case Management, the Business Office, Dining, Activities, Maintenance, and community leadership. Resident & Family Communication Develop professional and compassionate relationships with residents and their families. Communicate resident health concerns and changes clearly, respectfully, and within the appropriate scope of the position. Respond to resident and family questions or concerns promptly and ensure appropriate follow-through. Assist residents and families in understanding changes in care needs and services. Maintain appropriate professional boundaries. Protect resident privacy and confidentiality. Advocate for resident dignity, autonomy, choice, and rights. Required Qualifications Current and unrestricted New Jersey nursing license in good standing. Graduate of an accredited nursing program. Minimum of two (2) years of nursing experience in Assisted Living, Long-Term Care, Skilled Nursing, or a comparable senior-care environment required. Demonstrated experience caring for older adults and managing the clinical needs of a geriatric population. Experience with resident assessments, care planning, medication management, changes in condition, incident/fall response, and coordination of care. Working knowledge of documentation and regulatory expectations within an assisted living or long-term-care environment. Strong clinical judgment and ability to recognize and appropriately escal a changes in resident condition. Excellent verbal and written communication skills. Strong organizational, time-management, and follow-through skills. Ability to communicate effectively with residents, families, physicians, pharmacies, outside healthcare providers, frontline staff, and leadership. Ability to manage multiple priorities and respond appropriately to urgent situations. Ability to maintain resident confidentiality and exercise sound professional judgment. Preferred Qualifications Three (3) or more years of Assisted Living or Long-Term Care nursing experience. Previous nursing experience within a New Jersey Assisted Living Residence. Experience working with residents with Alzheimer's disease, dementia, or other cognitive impairments. Familiarity with New Jersey Department of Health assisted living requirements. Experience participating in regulatory surveys, clinical audits, quality-improvement initiatives, and/or plans of correction. Experience educating, coaching, or providing clinical guidance to frontline resident-care staff. Experience using electronic health records and electronic medication administration systems. Work Environment This position operates within an active assisted living community and requires frequent interaction with residents, families, employees, physicians, pharmacies, healthcare providers, and outside agencies. The Full-Time Nurse must be comfortable working in an environment where priorities can change quickly and must be able to balance scheduled clinical responsibilities with unexpected resident needs and emergencies.
