- PRN Only
- Licensed Practical Nurse in the state of Pennsylvania
- We are also open to graduate nurses awaiting licensure, pending available supervision
- Coordinate, assess, plan, deliver, and evaluate nursing care for assigned residents
- Perform accurate, thorough, and timely nursing assessments in accordance with department standards
- Deliver direct and indirect care that promotes each resident's highest level of functioning and well-being
- Evaluate residents' responses to treatment and collaborate with physicians and the interdisciplinary team to adjust care plans as needed
- Coordinate and document the discharge planning process, ensuring residents and families understand the ongoing care plan
