Social Work Services Program Consultant - 64082520

The Social Work Services Program Consultant functions as an integral member of the Children's Multidisciplinary Assessment Team (CMAT) and provides social work services and support to families and children served by the CMAT program. Your specific responsibilities include:

  • Completes day to day administrative operations which include responding to regional and statewide requests for information about CMAT coordination and staffing as well as raising the awareness of stakeholders regarding the team.
  • Reviews CMAT referrals for appropriateness, obtains all necessary paperwork. Communicates with stakeholders.
  • Completes psychosocial assessment including activities of daily living. Support opportunities for parental participation and family-centered approaches in the CMAT process.
  • Participate with the team to determine medically necessary eligibility of the client and family for services and about the availability, duration, and limits on possible supports and services in their program.
  • Participate in collaborative decision-making and problem-solving strategies with other members of the multidisciplinary team to determine individualized services to the care and management of the medical necessities of each client.
  • Communicate available information to CMAT members about the issues and policies which affect decisions and determinations of eligibility for care and management of the client's medical needs.
  • Encourage the placement of clients in the least restrictive setting appropriate to their medical needs and the wishes of their parents or legal representatives; completes and disseminates CMAT staffing summaries.
  • Complete additional CMAT activities within required timeframes. Document CMAT activities in the clients' CMS electronic medical record. Facilitate access to reports and records concerning the client being staffed when proper consent is available. Provide information from data base systems as requested by CMS Central Office.
  • Participate in training, evaluation, monitoring, and related follow-up activities to improve CMAT functions, activities, and outcomes. Ensures completion of level of care training for new CMAT members.

Required knowledge, skills, and abilities include:

  • Knowledge of medical terminology.
  • Knowledge of foster care system and child/family community resources.
  • Skill in performing psychosocial assessments.
  • Skill in detailed documentation.
  • Ability to communicate effectively verbally and in writing.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to access and utilize community resources.
  • Ability to access information from computer data systems and websites.
  • Ability to use a computer for word processing, spreadsheets, and database applications.

Qualifications:

  • Minimum: Must hold a valid unrestricted Florida Driver's License.
  • Preferred: Preference will be given to candidates with the following skills and experience. An Associate's degree or higher in Social Services. 2 years of medical setting and child welfare experience. Experience utilizing computer systems and applications in your daily work. Familiar with or experience working with medical terminology. Working knowledge or experience completing a comprehensive psychosocial and activities of daily living assessment.
