The hourly range is based on clustered visits: we schedule visits close together so nurses can complete up to 2 visits per hour.

What you’ll do

• Skilled nursing visits under the RN’s plan of care
• Medication administration visits, including insulin and GLP-1 injections
• Wound care, vital signs, medication setup and teaching
• Report changes in condition to the RN/clinical manager
• Document each visit the same day in our EMR
• Clock in and out with our visit verification (EVV) app at every visit

Requirements

• Active Massachusetts LPN license
• Current CPR/BLS
• Home health experience preferred; injections and wound care experience a plus
• Reliable car, valid driver’s license and car insurance
• Smartphone for visit verification; comfortable documenting on a phone or tablet
• Dependable, organized and comfortable working independently
