LCSW Social Worker

Job Summary:

The Licensed Clinical Social Worker (LCSW) will provide acute care social work services including discharge planning and care coordination across multiple hospital units. This role requires collaboration with interdisciplinary teams to support patients and families during hospital stays, ensuring smooth transitions of care. The position demands acute care experience, proficiency in Epic EMR, and strong case management skills in diverse clinical settings. Responsibilities:

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Coordinate discharge planning and facilitate appropriate post-acute care arrangements.
• Collaborate with medical staff, patients, and families to address barriers to care and support patient needs.
• Document case notes and patient interactions accurately in the Epic electronic medical record system.
• Provide crisis intervention and counseling as needed within the hospital setting.
• Work across multiple units including Medical/Surgical, Emergency Department, and Long Length of Stay (LLOS) patients.
• Participate in interdisciplinary team meetings to promote coordinated care delivery.
• Maintain compliance with hospital policies, state regulations, and professional social work standards.

Required Skills & Certifications:

• Master's degree in Social Work (MSW) with current Washington State LCSW license.
• Minimum of 1 year of acute care hospital social work experience.
• Proficiency with Epic electronic medical record system.
• Experience in discharge planning and care coordination.
• Basic Life Support (BLS) certification.
• Strong communication and interdisciplinary collaboration skills.

Preferred Skills & Certifications:

• Experience with AIDA EMR system.
• Diverse case management experience including Medical/Surgical, Emergency Department, and LLOS populations.
• Previous travel healthcare experience.

Special Considerations:

• Must be licensed and authorized to practice social work in Washington State.
• Work includes floating between hospital units including Colby and Pacific Campuses (less than 2 miles apart).
• Weekend work required every other weekend.
• Strict adherence to hospital scrub color policy (no reddish-orange permitted).
• Facility parking costs are covered.
• Guaranteed minimum of 72 hours per two-week period.
• RTO (Return to Office) restrictions must be disclosed at time of submission.
• Locals not accepted; must be a traveler with appropriate licensing.

Scheduling:

• Five 8-hour day shifts per week.
• Schedule includes every other weekend coverage.
• Shift assignment may vary depending on unit needs within the hospital system.
• Contract length is 13 weeks with immediate start preferred.
