Role Description This full-time, on-site Physical Therapist role is based in New York, NY and focuses on delivering high-quality, patient-centered care. Day-to-day responsibilities include evaluating patients, developing and implementing individualized treatment plans, and providing hands-on physical therapy and rehabilitation services. The Physical Therapist will monitor patient progress, adjust interventions as needed, and maintain accurate clinical documentation. The role involves collaborating with other healthcare professionals, educating patients and caregivers on home exercise programs, and supporting safe, effective recovery in home and community settings.\n

• Candidates should possess strong skills in Physical Therapy and Rehabilitation to assess, treat, and support functional recovery.

• Candidates should possess solid Medicine and Patient Care skills to deliver safe, evidence-based clinical services and manage diverse diagnoses.

• Candidates should possess Home Care experience and skills to provide effective therapy in home and community environments.
\n
• Active state licensure as a Physical Therapist and graduation from an accredited Physical Therapy program.
\n
• Strong communication, documentation, and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team.

• Ability to travel locally for patient visits, manage schedules efficiently, and maintain professional boundaries in home settings.

• Experience with orthopedics, neurology, geriatrics, or home health is preferred.
