Dietician/Health Coach - PT - Provo FM

Position Summary Perform office visits as a Health Coach, deliver weight loss and diabetes counseling, liaise with Health Department, manage social media, clerical, and scheduling duties. Build relationships with patients, provide education, collaborate with physicians and providers to improve protocols and patient outcomes, and assist with our value‑based care goals. Essential Job Functions Be a Health Coach/Registered Dietitian/Exercise/Nutrition/Weight Management professional. Knowledgeable in a variety of general health topics, specifically nutrition, physical activity, and chronic health conditions. Experience with health coaching/motivational interviewing. Take weight, blood pressure and other measurements; follow written and oral instructions; use computer programs to track patient information; answer phone calls. Possess good time management skills; be punctual and reliable. Attend monthly meetings. Qualifications Health Coach/Dietitian/Nutritionist/CHES – working with patients one‑on‑one on specific needs, educating patients individually on preventive general health, nutrition, food, complete motivational follow‑up, physical activity, and health behavior change. Interest and/or experience with health coaching/motivational interviewing. Punctual, organized, and manages time well. Spanish language skills would be useful but are not required.
