You will also assist consumers in assessing their or their care receiver’s needs for services, making decisions about service options, assessing benefits, and connecting with formal and informal providers. Qualifications Experience with client interviewing and conducting person‑centered assessments Well‑developed interpersonal communication, customer service, and problem‑solving skills Intervention and de‑escalation skills Knowledge of community resources, public assistance programs, and a basic understanding of Medicare Ability to use professional judgment to work independently Excellent time management Computer proficiency Work Schedule This position is Monday‑Friday, 8:00 AM to 4:30 PM. Employees are required to work one in‑office day per week; the remaining days may be worked from home.
