Description
  • Assess patients within both inpatient and outpatient environments and develop personalized treatment plans
  • Deliver effective physical therapy interventions aimed at improving mobility, strength, balance, and overall function
  • Work closely with nursing staff, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
  • Instruct patients and their families on therapeutic goals, planning for discharge, and at-home exercise programs
  • Maintain detailed records of evaluations, treatments, and progress in accordance with facility and regulatory standards
  • Engage in care team discussions and interdisciplinary meetings to ensure smooth care coordination
Education
  • Doctorate in Physical Therapy (DPT) or a recognized equivalent from an accredited institution
Certifications
  • State licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional specialized certifications (such as neurological or geriatric) are advantageous but not mandatory
Skills
  • Prior experience in a similar role is highly preferred
  • Capacity to treat patients with complex medical and mobility challenges
  • Strong interpersonal and collaborative skills
  • Proficiency with digital documentation and clinical record systems
