The Reliance Youth and Family Counseling Center is seeking a dedicated and compassionate Licensed Mental Health Therapist to join our integrated care team as we continue to expand our impact in the San Diego area. This full-time role is primarily remote, providing flexibility while enabling you to deliver critical mental health services to patients with complex medical and psychiatric needs. As a vital member of our outpatient office, your work will contribute significantly to the mental and emotional well-being of a diverse patient population.

Work Environment\nAs part of our integrated care team, you will work in a collaborative, remote setting with the potential for occasional in-office meetings. Your daily schedule will encompass Monday to Friday, from 8:00 AM to 5:00 PM, with occasional evening shifts depending on patient availability and needs. Key Responsibilities\n- \n
- Conduct thorough mental health assessments that evaluate the interplay between medical conditions and patients' psychological well-being, allowing for tailored treatment interventions. \n
- Develop and implement integrated treatment plans that address both psychiatric and medical needs, ensuring a comprehensive approach to patient care. \n
- Provide one-on-one and group therapy sessions using evidence-based techniques, adapting your approach to suit the unique situations and backgrounds of each patient. \n
- Engage in collaborative practice with medical providers to facilitate coordinated care and enhanced treatment outcomes for patients. \n
- Deliver crisis intervention and emergency support when necessary, maintaining a calm and supportive presence during critical situations. \n
- Educate patients on the links between mental health and physical wellness, equipping them with tools to manage their conditions effectively. \n
- Ensure accurate and confidential documentation of patient progress and treatment plans in compliance with legal and ethical standards. \n
- Actively participate in team meetings, case conferences, and interdisciplinary care planning sessions to contribute to the overall success of patient care initiatives. \n
- Stay updated on current research, best practices, and advancements in integrated mental health care to continuously improve service delivery. \n
Your work will focus on a diverse patient demographic, many of whom experience co-occurring disorders that complicate their mental health treatment. You will engage with individuals dealing with chronic pain, substance use disorders, and other medical challenges, requiring you to take a sensitive and informed approach to their unique situations. Your expertise will be pivotal in providing therapeutic support and guiding patients toward better health outcomes.
Qualifications\n- \n
- A Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution. \n
- Current and valid licensure as an LCSW, LPCC, or LMFT within the state of California. \n
- Demonstrated proficiency in addressing mental health needs of individuals with complex medical conditions, ideally gained through previous work experience. \n
- Proven ability to work collaboratively within interdisciplinary teams, particularly in settings that blend medical and therapeutic care. \n
- Strong interpersonal and communication skills, with the capacity to connect with diverse patient populations. \n
- Commitment to maintaining professionalism, ethical standards, and patient confidentiality. \n
- Familiarity with electronic health record (EHR) systems and applications relevant to mental health services. \n

- A minimum of 2 years of clinical experience in a mental health setting, ideally working with patients who have co-occurring medical and psychiatric disorders. \n
- Previous experience in integrated health care systems and familiarity with collaborative care models is highly desirable. \n
