Admissions Liaison – Skilled Nursing Facility

We are seeking an experienced and motivated Admissions Liaison to join our skilled nursing facility team. This role is critical in ensuring a smooth admissions process while delivering excellent customer service to residents, families, and referral partners.

Responsibilities

• Coordinate and manage the admissions process for new residents

• Communicate effectively with hospitals, families, referral sources, and internal teams

• Verify insurance benefits and coverage eligibility

• Review clinical documentation and ensure accurate data entry

• Maintain timely and accurate records in the EMR system

• Provide exceptional customer service to residents and families

• Collaborate closely with nursing, social services, and administration

Requirements

• Minimum 1 year of experience in a long-term care setting (required)

• Experience with EMR systems in long-term care (required)

• Strong customer service skills

• Excellent communication and interpersonal skills

• Experience with insurance verification and benefits

• Ability to work every other weekend (required)

• Strong organizational skills and attention to detail
