Admissions Liaison – Skilled Nursing Facility
We are seeking an experienced and motivated Admissions Liaison to join our skilled nursing facility team. This role is critical in ensuring a smooth admissions process while delivering excellent customer service to residents, families, and referral partners.
ResponsibilitiesCoordinate and manage the admissions process for new residents
Communicate effectively with hospitals, families, referral sources, and internal teams
Verify insurance benefits and coverage eligibility
Review clinical documentation and ensure accurate data entry
Maintain timely and accurate records in the EMR system
Provide exceptional customer service to residents and families
Collaborate closely with nursing, social services, and administration
Minimum 1 year of experience in a long-term care setting (required)
Experience with EMR systems in long-term care (required)
Strong customer service skills
Excellent communication and interpersonal skills
Experience with insurance verification and benefits
Ability to work every other weekend (required)
Strong organizational skills and attention to detail
