Skilled Nursing Facility (SNF) Program

Full-Time | Physician-Led Care Model

Position Overview
An established healthcare organization is seeking a compassionate and patient-focused Primary Care Physician (Internal Medicine, Family Medicine) or Geriatrician to provide comprehensive medical care to residents within Skilled Nursing Facilities (SNFs).

This role offers the opportunity to build meaningful relationships with patients while delivering high-quality, value-based care in a collaborative interdisciplinary environment. Physicians will focus on managing complex chronic conditions, post-acute care, preventive medicine, and care coordination for a predominantly geriatric patient population.

Responsibilities

• Provide comprehensive primary care services to SNF residents
• Conduct initial evaluations, follow-up visits, and annual wellness assessments
• Manage acute and chronic medical conditions
• Develop and oversee individualized treatment plans
• Coordinate care with nursing staff, therapists, specialists, and family members
• Participate in care plan meetings and interdisciplinary discussions
• Ensure accurate and timely clinical documentation
• Support quality improvement initiatives and value-based care objectives
• Provide compassionate, patient-centered care focused on improving outcomes and reducing avoidable hospitalizations

Qualifications

• MD or DO degree from an accredited institution
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active or eligible state medical license
• DEA registration or eligibility
• Experience caring for geriatric, post-acute, long-term care, or SNF populations preferred
• Strong communication and team collaboration skills
• Commitment to delivering high-quality, patient-centered care

Preferred Qualifications

• Geriatric Medicine fellowship training
• Previous Skilled Nursing Facility (SNF), Long-Term Care (LTC), or Post-Acute Care experience
• Value-based care experience
• Leadership or medical directorship experience
