Blumenthal Health & Rehabilitation Center in Greensboro, NC is seeking a qualified candidate to join our growing team as the Wound Care Nurse.

The Wound Care Nurse is responsible for coordinating, assessing, implementing, and evaluating the wound and skin care program for residents of the skilled nursing facility. This position collaborates with physicians, nurse practitioners, therapy, dietary, and nursing staff to promote optimal wound healing, prevent pressure injuries, ensure regulatory compliance, and improve resident outcomes. The Wound Care Nurse serves as the clinical resource for skin integrity and wound management throughout the facility. Responsibilities should be performed within the individual's scope of practice and consistent with North Carolina Board of Nursing guidance and facility policies.

• Perform comprehensive skin and wound assessments upon admission, readmission, significant change in condition, and as clinically indicated.

• Complete weekly wound assessments and measurements, including staging, drainage, tissue type, peri-wound condition, pain assessment, and healing progress.

• Develop, implement, and update individualized wound treatment plans in collaboration with the attending physician or wound care provider.

• Carry out or oversee advanced wound treatments as ordered.

• Monitor effectiveness of treatments and recommend revisions when appropriate.

• Identify residents at risk for pressure injuries and implement prevention interventions.

• Monitor pressure-relieving mattresses, cushions, heel protectors, and positioning devices.

• Ensure appropriate turning and repositioning programs are implemented.

• Participate in physician and wound care provider rounds.

• Monitor residents with surgical wounds, skin tears, diabetic ulcers, arterial ulcers, venous ulcers, pressure injuries, burns, ostomies, and other skin conditions.

Documentation

• Maintain accurate and timely documentation in the electronic medical record.

• Complete weekly wound measurements and progress notes.

• Ensure physician orders are current and accurately transcribed.

• Document resident response to treatments.

• Complete required skin assessments and Minimum Data Set (MDS) documentation as requested.

• Ensure documentation supports regulatory compliance and reimbursement.

Quality Assurance & Regulatory Compliance

• Monitor facility-acquired pressure injuries and identify trends.

• Participate in Quality Assurance Performance Improvement (QAPI) meetings.

• Review wound outcomes and develop action plans to improve healing rates.

• Assist during state surveys and other regulatory inspections.

• Ensure compliance with:

  • CMS Regulations

  • North Carolina State Operations Manual

  • Infection Prevention Standards

  • OSHA requirements

  • Facility policies and procedures

• Educate licensed nurses and CNAs on:

  • Pressure injury prevention

  • Skin assessment

  • Dressing application

  • Infection prevention

  • Off-loading techniques

  • Nutrition and hydration related to wound healing

• Participate in orientation of new nursing staff.

• Provide resident and family education regarding wound treatment plans.

Education

• Graduate of an accredited school of nursing.

Licensure

• Current unrestricted North Carolina RN or LPN license.

• Current CPR and AED certification.

Experience

• Minimum one year of long-term care experience.

• Previous wound care experience preferred.

• Wound care certification (WCC, WOCN, CWCN, or similar) preferred

Knowledge & Skills

• Knowledge of pressure injury staging.

• Knowledge of current evidence-based wound care practices.

• Excellent assessment and documentation skills.

• Strong communication and leadership abilities.

• Ability to educate staff effectively.

• Excellent organizational and time management skills.

• Ability to prioritize multiple tasks.

• Proficient with electronic medical records.

Physical Requirements

• Ability to stand and walk for extended periods.

• Frequent bending, kneeling, reaching, and lifting up to 50 pounds with assistance as needed.

• Ability to perform resident transfers utilizing proper body mechanics and lifting equipment.

• Manual dexterity sufficient to perform dressing changes and wound assessments.
