Job Overview

High Risk Physician

We are in search of Board Certified Internal Medicine or Family Medicine Physicians to join our High Risk Department . This role involves managing complex, high-risk patients with multiple chronic conditions who are transitioning in care or have acute, short-term needs. The physician will work closely with our medical team and care management staff to deliver medical care and assess the needs for care management, social work, and pharmacy services, aiming for optimal outcomes in quality metrics, patient experience, and cost-effectiveness.

Key Responsibilities:

• Work Monday through Friday with no weekend shifts and shared on-call duties. A four-day work week option is available.

• Positions are available at our New Britain location.

Qualifications:

• Must secure Connecticut licensure prior to starting practice.

• Board Certification and a DEA license are mandatory.
