Main duties of the job As a Specialist Rotational Pharmacist, you will: Collaborate with multidisciplinary teams on prescribing decisions. Undertake rotations in Oncology/Haematology, frailty and older person care, Urgent and Emergency Services, and other clinical areas. Monitor prescriptions and develop patient-centred pharmaceutical care plans. Ensure seamless pharmaceutical care through accurate drug histories and discharge planning. Actively contribute to medicines safety and quality improvement initiatives. Mentor and train other pharmacy and healthcare professionals. Participate in education and health promotion activities. Represent pharmacy in Trust meetings. Contribute to a weekday service with some weekend and on-call duties (home-based with initial screening). Current listing on the Pharmacist Register with the GPhC Diploma in pharmacy practice or equivalent experience Completed general level framework competency training Significant experience of working in a pharmaceutical environment Significant clinical pharmacy knowledge and experience Significant experience of working in hospital pharmacy Knowledge of national guidelines relating to medicines and pharmaceutical practice Specialist knowledge and experience in clinical speciality Independent prescribing Specialist training in Clinical Speciality Employer certification / accreditation badges You must have appropriate UK professional registration.
