Social Services Director Westgate Hills Rehabilitation and Nursing Center, a skilled rehabilitation and nursing facility in Havertown, Pennsylvania, is seeking a dedicated and compassionate Social Services Director to join our team. As a leader on our interdisciplinary team, you will play a critical role in promoting and protecting the rights and well‑being of our patients/residents. Responsibilities Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well‑being Develop and implement written care plans for each resident's social services needs Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers Establish and maintain relationships with community‑based agencies and services Qualifications Bachelor's degree from an accredited school of Social Work or related field required At least one year of supervised social work experience in a Skilled Nursing Facility, working directly with individuals, preferred Licensed in Social Work for the state of PA Benefits Competitive rate Medical, Dental, Vision Benefits SOFI Wellness - Daily Pay - 401K Match - Tuition Reimbursement About Us Westgate Hills Rehabilitation and Nursing Center is committed to providing Resident‑Centered Care as part of our Rapid Recovery Program.
