Gentiva is seeking a Hospice RN / RN Case Manager in Victoria, Texas. This position focuses on delivering personalized, meaningful hospice and palliative care to patients and their families facing life-limiting illnesses. You will be their primary point of contact, guiding them through every step of the hospice journey, while collaborating with an interdisciplinary team to ensure optimal care. The ideal candidate will have at least one year of RN experience in hospice or related fields, strong communication skills, and a compassionate approach to patient care.
