• ABOS Board Certified or TRULY Board Eligible in Orthopedic Spine Surgery (must become certified within 5 years) – REQUIRED * no exceptions
• Valid IL & IA license or Active IMLC Willing to Obtain Licenses - REQUIRED
• Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice- REQUIRED
• DEA - REQUIRED
• BLS Certification and maintain recertification - REQUIRED
• Clean malpractice and license history – HIGHLY PREFERRED / PREFERRED
• Does the provider require visa support – No J-1 or H-1B (Note in Additional Submission Details) – REQUIRED
• Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED
• BLS
• Board Certified
• Board Eligible

• $739K guaranteed (2 years); production after 10,650 wRVUs
• Straight production model year 3; earning potential $1M+
• $100K recruitment incentive + $15K relocation assistance
