Key Responsibilities\n- \n
- Facilitate educational and therapeutic group sessions to empower patients and families, documenting their goals and responses to promote constructive therapeutic activities. \n
- Participate in treatment planning meetings, engaging in collaborative discussions to evaluate patient progress and address any treatment resistance, while updating the treatment plans as necessary. \n
- Manage conflict and resistance in client interactions effectively, helping patients and families identify unproductive behaviors and develop healthier coping mechanisms. \n
- Maintain comprehensive and accurate patient records that adhere to established standards, documenting all assessments, treatment plans, interventions, and interdisciplinary communication. \n
- Demonstrate proficiency in DSM diagnostic criteria, addressing a range of disorders commonly found in children and adolescents, and apply this knowledge to tailor interventions. \n
- Gather and synthesize relevant patient data using effective interviewing techniques and consultation with healthcare professionals, ensuring a thorough psychosocial assessment. \n
- Create and regularly update individualized treatment plans based on ongoing assessments, incorporating patient and family concerns, strengths, and cultural values to provide comprehensive support. \n
Your primary focus will be on patients receiving hospice care, who may be facing terminal illnesses. This role requires an understanding of their complex emotional and psychosocial needs, as well as the unique challenges that families encounter during this sensitive time. You will also work closely with healthcare providers to facilitate a supportive environment that respects the dignity and preferences of each patient.
Qualifications\n- \n
- A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline. \n
- Must hold an active and independent license as a Licensed Clinical Social Worker (LCSW). \n
- Exceptional communication and interpersonal skills, fostering trust and rapport with clients and their families. \n
- Strong organizational skills, ensuring timely and accurate documentation of client interactions and treatment plans. \n
- Ability to interpret a variety of information types, including written documents, oral instructions, and treatment diagrams. \n
- Current Basic Life Support (BLS) and CPR certifications. \n

- Experience working in hospice or palliative care settings is strongly preferred. \n
- Proficiency in utilizing electronic health record (EHR) systems for documentation and communication. \n
- A background in conducting evidence-based therapeutic practices, including behavior modification and assertiveness training, is advantageous. \n
