Licensed Behavioral Therapist (Consultant)- LCSW, LPC

Job Summary This role is a core member of the integrated care team. It provides short‑term, individualized clinical interventions to members identified as seeking therapy and having a behavioral health condition that negatively impacts their overall health and daily functioning. The approach is trauma‑informed, emphasizing harm reduction and motivational interviewing to elicit behavioral change and enhance adherence to medical recommendations and treatment plans. This role also serves as the behavioral health subject‑matter expert within an interdisciplinary care team that includes primary care providers, nurses, medical/social work care managers, psychiatrists, pharmacy, community health workers, and resource specialists. The behavioral health clinician may recommend levels of care not offered through AbsoluteCare providers, such as ACT, IOP, PHP, residential treatment, or inpatient services. The clinician comprehensively assesses members with a biopsychosocial approach, develops person‑centered treatment plans with measurable SMART goals, and provides a combination of short‑term individual therapy, group therapy, and post‑hospital follow‑up within seven days of discharge from an inpatient unit. Key performance indicators include meeting treatment plan goals, increased adherence, decreased preventable medical/psychiatric events, improved HEDIS post‑hospital discharge measures, and member retention. The clinician promotes the mission, vision, and values of AbsoluteCare in all interactions with members and multidisciplinary team members. Duties and Responsibilities Provide individual, group, and post‑discharge follow‑up behavioral health therapy, including comprehensive biopsychosocial assessments, individualized treatment plans using SMART goals, timely clinical documentation (e.g., SOAP notes), and tracking progress toward goal completion. Provide crisis intervention and de‑escalation for members presenting in crisis (via telephone or in person), including suicidality, disruptive or threatening behaviors, and disposition as clinically appropriate. Serve as the clinical behavioral health SME within the interdisciplinary care team by providing recommendations, referrals, and follow‑up for behavioral health aspects of member care. Advocate for and provide clinical care using evidence‑based approaches, including trauma‑informed care, harm‑reduction, motivational interviewing, cognitive‑behavioral therapies, and brief solutions‑focused or emotional management interventions such as anger management. Maintain close communication with primary care providers and psychiatrists for all members receiving therapy. Complete documentation of patient care services in the electronic health record within three business days of interaction. Develop and/or support individual treatment plans for members with chronic mental illness, including schizophrenia, bipolar disorder, depression, and anxiety. Recognize opportunities to promote long‑acting injectable medication when appropriate to support treatment adherence. Schedule appointments, promote attendance, and manage barriers to attendance (e.g., transportation) through reminder calls and follow‑up after missed appointments. Collaborate with clinical leadership to develop and maintain a group therapy curriculum tailored to the member population, covering educational and process components such as grief, anger, anxiety, aftercare, overall health and wellness, and tobacco cessation. Minimum Qualifications Clinically licensed in the state of practice with 3+ years of experience in a behavioral health related field (e.g., social work, mental health, addiction). Licenses permitting clinical practice, such as LCSW or LPC. At least 2 years of experience in a community mental health center, FQHC, inpatient psychiatric facility, or another setting serving clients with complex behavioral health needs. Experience with complex government‑sponsored populations (Medicaid, Medicare) preferred. Experience with member engagement, transitions of care, interdisciplinary clinical care, and/or case management preferred. Telehealth experience preferred. Excellent clinical judgment and ability to independently manage a caseload. Advanced computer skills with proficiency in Outlook, Word, PowerPoint, and Excel. Excellent written and oral communication skills, including the ability to adapt communication to the member’s needs and comprehension level. Experience working in multiple electronic health records. Excellent organizational and time‑management skills to triage and prioritize patient needs. Proven ability to work within a fast‑paced, dynamic environment. Active CPR certification. Working Conditions This job operates in a professional office environment and routinely uses general office equipment. Physical Requirements Communicate clearly and exchange accurate information consistently. Remain stationary for long periods of time. Use a computer, keyboard, copier, fax machine, phone, and other general office equipment. Occasionally move objects up to 20 lbs. Direct Reports None.
