Nurse - Quality Facilitator

Summary The Quality Facilitator Registered Nurse (RN) position is located in the Quality and Patient Safety service line at the Chillicothe VA Medical Center, Chillicothe, Ohio. The Quality Facilitator RN is responsible for documented outcomes at the program or service level for the Medical Center and all off‑site locations within the Integrated Clinical Community (ICC). Qualifications Basic Requirements: English Language Proficiency: In accordance with 38 U.S.C. 7403(f), a nurse must be proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency (ACEN or CCNE). OR individuals attending a master’s level bridge program in nursing with coursework equivalent to a bachelor’s‑level degree may be appointed as an RN prior to completion of the bridge program. Upon achieving a state license, the individual may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Foreign‑school graduates: possession of a current, full, active, and unrestricted registration from a U.S. state, territory, commonwealth, or the District of Columbia, or a formal degree equivalency validation from a recognized evaluator such as International Consultants of Delaware (ICD). Grade determinations: Role competency and education level are matched to the appropriate grade level (e.g., Nurse I, II, III) based on the criteria documented in the online assessment. Preferred experience: Strong clinical background in Med‑Surg and specialty clinic areas. Experience in accreditation, standard reviews, directives, policies, and standard operating procedures development and compliance. Physical requirements: ability to sit or stand for several hours daily while interacting with patients, employees, and providers via computer or phone; ergonomic workspace and adequate vision and hearing. Duties The Chillicothe VA Medical Center provides primary care and specialty health services, including mental health, spinal cord injury treatment, women's health, nutrition counseling, social work, and more. Collect data, monitor staff compliance with relevant policies, directives, and standards. Provide reports detailing trends and offer direct staff support across services in planning, designing, integrating, implementing, modifying, and administering a comprehensive quality management program. Participate in facility councils, committees, and workgroups. Work with leaders and others to sustain safety and model High Reliability Organization (HRO) practices. Support facility leadership in planning, organization, development, direction, and evaluation of the quality program within the facility. Communicate and collaborate with the interdisciplinary team to analyze findings, identify opportunities for improved quality of care, and analyze clinical content of records for quality and appropriateness.
