We are ready for you to join us./ppLicensed Vocational Nurse Responsibilities:/pulliUnder the supervision of a registered nurse, provides quality care to patients and performs technical skilled care in patients home in compliance with nursing standards, federal, state, local government regulations, and company clinical policies./liliProvides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or a Registered Nurse designee./liliEnsures quality and safe delivery of home health services to patients/clients and families/caregivers./liliImplements a plan of care formulated by a physician and Registered Nurse./liliProvides accurate and timely documentation consistent with the plan of care./liliAssesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care./liliMaintains a safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards./liliParticipates in coordination of home health services with Registered Nurse and clinical management./liliAssists the patient in learning appropriate self-care activities/liliUses equipment and supplies effectively and efficiently./liliDocuments and reports activity as required./li/ulpQualifications:/pulliState registered nurse license required/lili1-2 year of experience in in-home care/liliCOVID vaccination preferred/liliExcellent communication and time-management skills/liliCompassionate and friendly demeanor/liliAbility to travel to multiple patients per day/liliPreferred knowledge of Wellsky/Kinnser and HCHB/li/ul/div
