Aventura at Walton Hills is seeking a compassionate, motivated, and licensed Physical Therapist Assistant (PTA) to join our full-time, in-house rehabilitation team . If you're passionate about helping residents achieve their highest level of independence while working in a supportive Skilled Nursing and Long-Term Care environment, we'd love to hear from you!

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.

• Assist residents in improving mobility, strength, balance, endurance, and functional independence.

• Accurately document treatments, resident progress, and outcomes using Net Health .

• Collaborate with physicians, nursing staff, therapy colleagues, and interdisciplinary team members to deliver exceptional resident-centered care.

• Maintain compliance with all federal, state, facility, and professional practice standards.

• Educate residents and families on treatment plans, safety techniques, and home exercise programs as appropriate.

Qualifications

• Current Ohio Physical Therapist Assistant (PTA) license in good standing.

• Skilled Nursing Facility (SNF) or Long-Term Care experience preferred.

• Experience with Net Health preferred.

• Excellent communication, organizational, and teamwork skills.
