Registered Nurse (RN) - Home Health

Location: Lawrence & Morgan County, Alabama
Schedule: Monday - Friday, 8:00 AM - 4:30 PM
Hours: 40 hours per week
Productivity Expectation: 30 points per week
On-Call/Weekend Requirement: As requested by the agency

Position Overview

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team serving patients throughout Lawrence and Morgan Counties . In this role, you will provide high-quality nursing care in patients' homes, helping them manage acute and chronic health conditions while promoting independence and improving overall health outcomes.

The ideal candidate is clinically strong, organized, and comfortable working independently in a home health environment.

Key Responsibilities

Perform comprehensive patient assessments and develop individualized care plans.
Provide skilled nursing services in patients' homes according to physician orders and agency guidelines.
Deliver specialized wound care and monitor healing progress.
Administer and manage IV therapy and IV care as required.
Monitor patient conditions, evaluate treatment effectiveness, and communicate changes to the interdisciplinary care team.
Educate patients and caregivers regarding disease management, medications, treatments, and safety practices.
Complete timely and accurate clinical documentation.
Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate patient care.
Ensure compliance with all regulatory, agency, and quality standards.

Qualifications

Required

Active Alabama Registered Nurse (RN) License in good standing.
Experience with wound care and IV care/therapy .
Strong clinical assessment and patient education skills.
Valid driver's license and reliable transportation.
Ability to travel throughout the assigned coverage area.

Preferred

Previous home health nursing experience .
Experience utilizing Homecare Homebase (HCHB) .
Strong time management, organizational, and communication skills.
