This position is responsible for providing medical services directly to the clients in the Suburban Health program. This includes maintaining the appropriate records and medical files as per state, federal and agency regulations. This position is responsible for the planning, implementation and/or coordination of all medical treatment services delivered to clients that are assigned. This includes ensuring that established procedures are followed and clinical standards that are set for treatment planning, the delivery of medication services, and discharge planning and referral services are met. Significant creativity and innovation are necessary in order that clients are successfully engaged, motivated and supported in their efforts to achieve treatment plan objectives. Collaboration and coordination with both internal and external resources are crucial to successful client outcomes. TASKS AND RESPONSIBILITIES: Medical Coordination: Responsible for the medical coordination which establishes a framework of action to enable the client to achieve specified goals. It involves collaboration with the physician, client and significant others, coordination of treatment and referral services, liaison activities with community resources and managed care systems, client advocacy, and ongoing evaluation of treatment progress and client needs. Prior to admission, reviews medical history to ensure that Suburban Health can meet client’s needs; Ensures that the medical certification is completed prior to client’s admission into the program; Arranges physical to be done by the physician; Arranges for all laboratory tests to be completed per physician’s order; Administers and reads all TB testing; Collaborates with the physician to evaluate ongoing medication needs; Ensures that adequate recording of the Administration/dispensing transaction has occurred; Reviews the medication of the client, prepares the medication sheets, sets the schedule for administration; Ensures that the client is aware of the dangers as well as the benefits of medication management; Documents all medication and progress in each client’s record; Monitors patient response to treatment and medication dosage when they come for medication for signs of over-sedation and/or illness; Ensures that treatment plans reflect medication compliance as well as any chronic medical condition and it’s treatment; Ensures that medications are being ordered from the pharmacy as needed and that the amount of stock drugs meet the amount established as “par” level; Ensure that medications, stock drugs and lab and medical supplies are stored properly Treatment Planning: Responsible for the collaborative process in which the medical team and the client develop a written document that identifies important treatment goals, describes measurable, time sensitive action steps toward achieving those goals with expected outcomes and reflects an agreement between counselor and client. Guides the treatment planning process with the use of relevant assessment information; Examines treatment options in collaboration with the counselors, client and significant others (when appropriate); Formulates mutually agreed upon and measurable treatment goals and objectives; Identifies appropriate strategies for each treatment goal; Develops with the client a mutually acceptable treatment plan and method for monitoring and evaluating progress; Reassesses the treatment plan at regular intervals or when indicated by changing circumstances. Counseling: Responsible for the collaborative process that facilitates the client’s progress toward mutually determined treatment goals and objectives. Counseling includes methods that are sensitive to individual client characteristics and to the influence of significant others, as well as the client’s cultural and social context Establishes a helping relationship with the client characterized by warmth, respect, genuineness, concreteness, and empathy; Facilitates the client’s engagement in the treatment and recovery process; Promotes client’s knowledge, skills, and attitudes that contribute to a positive change in substance use behaviors; Encourages and reinforces client actions determined to be beneficial in progressing toward treatment goals; Works appropriately with client to recognize and discourage all behaviors inconsistent with progress toward treatment goals; Adapts counseling strategies to the individual characteristics of the client, including but not limited to disability, gender, sexual orientation, developmental level, culture, ethnicity, age, and health status; Makes constructive therapeutic responses when the client’s behavior is inconsistent with stated recovery goals. Internal and External Communication/Collaboration: Responsible for facilitating the client’s use of available support systems and community resources to meet needs identified in clinical evaluation and/or treatment planning. Additionally, responsible for ensuring that the treatment team is aware of the client’s treatment goals, recovery process and to receive feedback from the treatment team as well as offering insights and feedback to the team as it relates to all agency participants. Contributes as part of the multidisciplinary team; Applies confidentiality rules and regulations appropriately; Demonstrates respect and non-judgmental attitudes toward clients in all contacts with community professionals and agencies; Exchanges relevant information with the agency or professional to whom the referral is being made in a manner consistent with confidentiality rules and regulations and generally accepted professional standards of care. Administration: Responsible for the accurate, timely and complete documentation of medical services personally delivered, as well as for ensuring that the documentation of other services accomplished is complete. This position is also responsible for issuing reports and communications that are called for in the completion of assigned duties. Responsible for the efficient operation of the site in conjunction with other team members and ensures that all client activities occur as planned. Completes all treatment plans, progress notes, group notes, and any other documentation according to policy, which is legible, and reflects client participation in treatment; Ensures that all reports and/or documents requested by management are accurate and timely; Ensures efficient programming occurs through volunteering coverage as needed; Anticipates the needs of the clients and the program in order to ensure effective programming. Professional Responsibility: Responsible for adhering to all applicable Federal and State laws, the professional code of ethics prescribed by the Nursing Board of New Jersey; the policies and procedures detailed in the Suburban Health Policy and Procedure Manual, the Suburban Health Code of Ethics statement and the instruction provided by supervision to include alternative duties. This includes the responsibility to make no statement, or take any action that would knowingly threaten the best interests of the agency, other staff members, and/or of clients in our care. The holder of this position is also responsible for undertaking and sustaining a commitment to job related professional growth. Adheres to all confidentiality laws as applied to substance use; Adheres to the professional code of ethics of the agency and the Nursing board; Demonstrates awareness of and competency in cultural diversity; Develops personal & professional goals developed in conjunction with supervisor to improve the effectiveness and efficiency of programming. EDUCATION: Completion of an undergraduate nursing program or nursing school program Registered Nursing license in NJ preferred Licensed Practical Nurse in NJ EXPERIENCE: At least one-year full-time experience or the full-time equivalent with the management of addictions in a licensed substance abuse treatment facility Possess the knowledge and understanding of how to recognize and manage an opiate overdose Understand the clinical benefits and limitations of urine screens, toxicology, and other testing procedures Knowledge of 12- Step Recovery Program theory preferred QUALIFICATIONS: Organizational and administrative skills Able to communicate effectively (both written and oral skills) Ability to work independently Ability to work under pressure Familiarity with health and safety issues Two years Sobriety prior to application (if applicable).
