Responsibilities:

• Evaluate patients’ needs and develop personalized treatment plans.
• Implement therapeutic interventions to improve patients' daily living skills.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Educate patients and their families on occupational therapy processes.
• Maintain accurate documentation of patient care and progress.
• Provide support and guidance to patients in their recovery journey.
• Stay updated with the latest Physical therapy practices and regulations.

Requirements:
Occupational Therapy from an accredited institution.

• Current state licensure to practice as an Physical Therapist.
• Strong communication and interpersonal skills.
• Experience in home health care preferred.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a patient-centered approach.
• Solid organizational skills and attention to detail.
• Willingness to travel to patients' homes as needed.
