Details Home Health setting Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM Caseload of approximately 4-5 patients per day with 5 weighted visits per day Patient population includes geriatrics, neurology, and orthopedic conditions Requirements Active CA PT license required (license required if applicable) Minimum 1 year of experience as a Home Health PT required Valid driver's license and auto insurance required BLS certification preferred Experience with OASIS documentation preferred Experience with home exercise programs, patient/family education, and safety/injury prevention Additional Information Responsibilities include assessment, evaluation, and treatment planning for home health patients Skilled in orthopedic, neurologic, and cardiopulmonary conditions including post-op care, stroke rehabilitation, and joint replacements Experience with adaptive and assistive devices, gait training, and therapeutic modalities Familiarity with billing under Medicare, Medicaid, commercial, and private pay preferred Local candidates accepted at local rate within a 50-mile radius Maximum of 7 days RTO per contract, with only 1 holiday request permitted per contract Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day American Traveler Job ID P-720582.
