MDS Nurse- Flexible Hybrid Hours!

MDS Nurse Opportunity

Hudson Park Rehabilitation and Nursing has a fantastic full time opportunity for a dynamic MDS nurse. This role includes flexible hours with hybrid remote options. The MDS Coordinator is responsible for the accurate and timely completion of the Minimum Data Set (MDS) assessments in accordance with federal and New York State regulations.

MDS Nurse Responsibilities:

• Assessing and monitoring patients' health in accordance with medical laws, regulations, and guidelines.
• Collecting patient data and organizing the information into reports for further research.
• Communicating with health care teams to create applicable health care plans for patients.
• Assisting and alerting patients with any notice of non-medical coverage updates

MDS Nurse Requirements:

• Complete and coordinate all MDS assessments (OBRA and PPS) in compliance with CMS RAI Manual requirements and NYS DOH regulations
• Lead and facilitate interdisciplinary care planning meetings; ensure care plans are individualized, goal-oriented, and updated per regulatory timelines
• Collaborate with therapy, nursing, dietary, social work, and activities departments to gather and validate MDS data
• Maintain the facility assessment schedule
• Monitor MDS accuracy, quality indicators (QMs), and Five Star ratings; identify trends and recommend clinical improvements
• Educate nursing and ancillary staff on MDS documentation requirements and clinical capture
• Ensure compliance with all HIPAA, NYS DOH, and federal skilled nursing facility regulations
• NYS RN license.
• Solid knowledge of MDS processes such as the Resident Assessment Instruments (RAI).
• Excellent organizational and multitasking skills.
• The ability to make independent judgments utilizing initiative, discretion, and the nursing process, as needed.
