Overview We are hiring Nurse Practitioners for in-home clinical assessments. About Us Matrix Medical Network is a national in‑home clinical network that delivers comprehensive health assessments across the United States. Our clinicians provide personalized care, improve health outcomes, lower costs, and empower patients to thrive. We serve older adults and other at‑risk individuals, often in their homes or skilled nursing facilities. Responsibilities During an approximately one‑hour visit, providers conduct a comprehensive health assessment (CHA). Key tasks include: Conduct adult/geriatric assessments covering medical history, diagnoses, treatment, health education, physician and case‑management referrals, and documentation per Matrix guidelines. Administer and order point‑of‑care and lab‑based testing as needed. Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel. Work with the Primary Care Physician (PCP) on patient education and follow‑up. Provide services at the member’s residence, including homes and skilled nursing facilities. Benefits of Working at Matrix Flexibility: Sessions are scheduled from 8 am to 7 pm, Sunday through Saturday. Schedule your visits around your availability. Competitive Pay: Income is based on completed assessments. Monthly earnings range from $1,800 to $11,500 based on visit volume (20–100 visits) and per‑assessment rates of $90–$115, which vary by state. Education and Support: Comprehensive onboarding, a preceptor, one‑on‑one feedback, and free continuing education access. Additional Licensure: Support for obtaining licensure in neighboring states if needed. Full‑time Opportunities: PRN providers can transition to full‑time roles in their home location, a state, or a region, with a sign‑on bonus. Qualifications Master’s Degree or equivalent experience and successful completion of NP licensure. Current NP licensure in state of practice, including prescription authority (or the ability to obtain it). Board certification by AANP, AACN, or ANCC in one of the following: Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. One year of NP experience preferred, though new graduates are encouraged to apply. Valid state driver’s license for the state of residence and automobile insurance coverage for that state. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills. Insurance requirements for bodily injury and property damage: $100,000 per person / $300,000 per incident; $50,000 property damage; $1,000 or less deductible.
