The Licensed Social Worker performs comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. What you will receive: Full‑Time, 32 Hour Position CEU Courses Educational Programs Growth/Advancement Opportunities No Weekends or Holidays Required Supportive Leadership & Work Wellness Responsibilities Conduct comprehensive evaluations, assess the social, emotional and support needs of patients and their families, and set measurable objectives and goals in collaboration with the rehabilitation team. Adjust treatment plan to achieve maximum results in independence and/or function. Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilize special skills and judgment to resolve adverse social and emotional responses associated with the injury/illness. Provide direct social work intervention and instruct and counsel patients and family members in coping with social and emotional responses related to the injury/illness. Regularly reassess effectiveness of treatment plan, progress of patient toward goals, and need for modifying goals or treatment. Communicate patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable. Consult with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment. Observe, document and report changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members. Maintain and submit evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations. Adhere to standards of the Company’s Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws. Perform other duties as assigned. Qualifications Master’s Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC). Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice required by individual states. A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis. Current BLS (Basic Life Support) First Aid and CPR certifications. Demonstrates effective and professional interpersonal, verbal and written communication skills. Able to work independently and part of a team. Demonstrates strong organization skills and attention to detail. Additional Job Information Ability to lift fifty (50) pounds. Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds. Ability to stand for extended periods. Fine motor skills. Visual acuity. Duties require extensive standing and walking. Requires lifting, positioning, pushing, and/or transferring patients.
