Shift Full time Days Care Connections Nurse Role Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. Care Connections Nurse will work with the patient to focus on collaboration between hospital and community‑based providers and arranging post‑discharge services. Care Connections Nurse functions as a member of a multidisciplinary team with a goal of improving the patient's health status and reducing the need for hospital care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students. Job Requirements 1 year applicable experience Basic Life Support certification by American Heart Association or Red Cross Registered Nurse credentials of any type Bachelor’s degree in Nursing or related field Benefits Collaborative environment where all voices are heard.
