Pediatric Home Health Nurse — Impactful & Autonomous

Job Summary The Pediatric Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education. Location Bronx, NY Job Type Fee‑for‑Service, paid per visit Pay $50–$125 per visit. SOC $100 per visit with an extra $25 bonus if notes are completed within 24 hours. Follow‑up/Revisit $50 per visit with an extra $25 bonus if notes are completed within 24 hours. Responsibilities Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Regularly re‑evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops the plan of care with the patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions and includes the patient and family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient/family as appropriate per assessment and plan. Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient. Knowledge of high‑tech nursing skills such as indwelling or straight catheter insertion/removal, nephrostomy care, ostomy care, peristaltic and abdominal catheter drainage care, infusion services, G‑tube and negative pressure wound therapy. Working knowledge of all infection control procedures and safety precautions. Knowledge of the nurse’s scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process. Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re‑evaluate patient needs through physical assessment and response to therapy and any updated physician orders. Inform physician of any changes in patient condition, response to care and progress. Obtain and clarify orders for Plan of Treatment revisions. Prepare clinical notes and update the primary physician when necessary and at least every sixty days. Complete and submit all documentation in accordance with policy and procedures. Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physician orders as required. Communicate with other health‑related persons to coordinate care plan. Participate in on‑call duties as defined by the on‑call policy. Ensure that arrangements for equipment and other necessary supplies and services are available. Instruct, supervise and evaluate home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instruct, supervise and evaluate LPN/LVN care provided at least every 30 days. Exercise professional management and coordinate care between and among involved parties in home, community providers and pediatric tertiary specialists. Qualifications Bachelor’s degree in Nursing from an accredited college/university. Current active and unrestricted nursing license. Pediatric care experience in the acute and/or home care environment. Valid driver’s license. Current certification in Basic Life Support for Health Care Professionals. Minimum of one year of recent acute or home care experience, preferably in an institutional setting. Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, nursing skills per competency checklist.
