Schedule: Monday–Friday, Day Shift
Guaranteed Hours: 36 Hours per Week

GSK Healthcare & Services is seeking a compassionate and dedicated Home Health Registered Nurse (RN) for a 20-week contract opportunity in Vermont.

The organization provides high-quality home health, hospice, and other community-based healthcare services, ensuring compassionate and accessible care for patients from all backgrounds.

As a Home Health RN, you will provide individualized, goal-oriented nursing care in accordance with physician orders and established nursing standards. You may also serve as a Case Manager when designated as the lead discipline, coordinating patient care and supporting positive health outcomes.

Responsibilities

• Provide individualized nursing care to patients in their homes.

• Assess patients' health conditions and develop appropriate nursing care plans.

• Implement and evaluate care plans in accordance with physician orders.

• Monitor patient progress and communicate changes in condition to the healthcare team.

• Coordinate care and serve as Case Manager when assigned as the lead discipline.

• Maintain accurate and timely patient documentation.

• Educate patients and families on health management, treatment plans, and disease prevention.

• Collaborate with physicians and other healthcare professionals to ensure continuity of care.

Requirements

• Active Registered Nurse (RN) license in Vermont or applicable compact license.

• Minimum of 1 year of nursing experience.

• Current BLS/BCLS certification.

• Home Health or Hospice nursing experience is preferred.

• Strong patient assessment, communication, and care coordination skills.

• Ability to work independently and provide compassionate, patient-centered care.
