Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience required.
• Experience with HCHB (Homecare Homebase) required.
• Experience performing SOC, ROC evaluations, recertifications, and revisits required.

Preferred Qualifications

• Previous travel therapy experience preferred.
• Experience working in St. Joseph and Marshall County home health markets preferred.

Assignment Details

• Desired territory: St. Joseph and Marshall County.
• Key duties include SOC, ROC evaluations, recerts, and revisits.
• Expected hours/points: 27 points per week.
• Candidate must be able to work in a home health environment and complete documentation in HCHB.
