Hospice RN Admissions Nurse

Addus Family Company is hiring a Hospice RN Admissions Nurse to support patients and families at the start of hospice care. In this role, you will conduct in-home admissions assessments, verify eligibility, obtain consents, and develop initial plans of care aligned with hospice regulations. You’ll educate patients and caregivers about hospice services, pain and symptom management, and safety in the home, collaborating closely with the interdisciplinary team.
