We are currently seeking a Care Plan Coordinator .
Job Responsibilities and Duties:- Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
- Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
- Provide guidance and support to nursing staff regarding patient care and documentation practices.
- Unencumbered RN or LPN license
- 2–5 years of experience in long-term care
- Knowledge of Medicare regulations
- Commitment to Total Quality Management
- Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing
