This position provides emotional support, mental health evaluations and therapy, and case management services.CORE JOB FUNCTIONS* Advocates for and helps clients gain access to resources while confronting their personal issues.* Counsels and provides crisis intervention for patients, including assessment and treatment of behavioral problems.* Coaches patients on how to manage ongoing health conditions.* Provides therapy to help patients make positive behavior changes.* Assists patients and their families through individual, group, and education conferences to understand, accept, and follow treatment recommendations.* Refers patients and their families to specialized treatment programs in accordance with their needs, and monitors and evaluates their progress.* Collaborates with the other departments to integrate care where needed.* Maintains current and appropriate documentation in patient chart.* Acts as a mandated reporter in suspected child abuse cases.* Adheres to University and unit-level policies and procedures and safeguards University assets.This list of duties and responsibilities is not intended to be all-inclusive and may be expanded to include other duties or responsibilities as necessary.Department Specific Functions:Provides psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.Administers a broad variety of therapies and is the designated expert, consultant, or source of knowledge within the social work profession.Formulates, develops, and implements a plan utilizing appropriate social work interventions.Collaborates with the health care team and involves the patient/family in the development and implementation of plans.Facilitates discharge planning across a multidisciplinary care continuum which includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient.Provides education and counseling to patients and families around issues related to adaptation to the patient's illness and/or life situation.May utilize crisis intervention, brief and long-term individual, group, and family therapies.Supervises Master of Social Work (MSW) students.May function as a resource to other staff as appropriate.Provides education to other health care professionals.Participates in or consults on research/education programs.CORE QUALIFICATIONSEducation:Master's degree in Social WorkExperience:Minimum 1 year of relevant experience requiredCertification and Licensing:Valid State of Florida licensed clinical social workerRefer to department description for applicable certification requirementsKnowledge, Skills and Abilities:Learning Agility: Ability to learn new procedures, technologies, and protocols, and adapt to changing priorities and work demands.Teamwork: Ability to work collaboratively with others and contribute to a team environment.Technical Proficiency: Skilled in using office software, technology, and relevant computer applications.Communication: Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.This is a core job profile description and is not reflective of all duties that may be assigned to a specific position in each individual department.
