Wellness Specialist - Community Mental Health & Recovery

S:US in New York is seeking a Wellness Specialist to join the ACT Team, delivering wellness management services and coordinating linkage to health, housing, entitlements, education and vocational programs to support successful community placement.
You will provide individual and group services alongside a multidisciplinary team, participate in daily meetings, and assist with housing attainment, social relationships, and recovery from mental illness in a client-centered, trauma-informed approach.
