UnityPoint Health is seeking an RN Case Manager - Home Health in the Waukee/Grimes/Johnston area. You will provide one-on-one care with total autonomy over your role, managing patient admission, care plans, and discharge in patients' homes. You will conduct clinical assessments, develop and oversee home health plans under physician direction, and collaborate with the interdisciplinary team to achieve high-quality outcomes and exceptional patient experience.
