Full Time: 3-11
Part Time: 7am-7pm or 7am-3pm, 3-11
Job Summary:
Uniontown Nursing and Rehab, a community-centered organization for over 30 years, is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As a member of our evening shift, you will play a vital role in providing exceptional care to our residents, fostering a sense of community, and making a meaningful impact on the lives of those we serve.
Responsibilities:
Provide direct patient care, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans, in collaboration with the interdisciplinary team
Monitor resident progress, reporting any changes or concerns to the nursing staff
Maintain accurate and detailed records of patient care and treatment
Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination
Participate in performance improvement initiatives, contributing to continuous quality improvement

As a Licensed Practical Nurse (LPN), you will have the opportunity to work closely with our experienced nursing staff to provide high-quality care to our residents.
