Part Time: 7am-7pm or 7am-3pm, 3-11

Job Summary:

Uniontown Nursing and Rehab, a community-centered organization for over 30 years, is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As a member of our evening shift, you will play a vital role in providing exceptional care to our residents, fostering a sense of community, and making a meaningful impact on the lives of those we serve.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans, in collaboration with the interdisciplinary team

• Monitor resident progress, reporting any changes or concerns to the nursing staff

• Maintain accurate and detailed records of patient care and treatment

• Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination

• Participate in performance improvement initiatives, contributing to continuous quality improvement
