Position Overview Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover in their own homes. The role involves performing assessments to evaluate mobility, strength, and function, and developing individualized plans of care with clear goals and outcomes. Responsibilities include delivering prescribed therapy services, documenting patient progress, modifying treatment plans as needed, and managing discharge planning with patient and caregivers. Qualifications & Skills Degree in Physical Therapy from an accredited program Current, unrestricted state license as a Physical Therapist Valid CPR, Driver’s License, and proof of automobile insurance Preferred Experience in home health therapy and OASIS documentation.
