Pharmacist

This position functions within the pharmacy department. Its primary purpose is to assure efficacious, safe, and cost-effective drug therapy and appropriate outcomes in the use of drugs and medicines. Responsibilities include preparing and distributing medications according to Federal and State law. Providing patient counseling and education is essential. The pharmacist will advise medical, nursing, and village staff on drug therapy. Selecting and maintaining an appropriate drug inventory is necessary. The pharmacist will formulate and participate in quality improvement activities. The services directly impact the healthcare of patients. The role requires independent professional judgment prior to the filling and dispensing of each prescription. It is necessary to evaluate each prescription for appropriateness and safety. Consultation with the prescriber is required if any questions arise, along with recommendations for alternative therapy. Judgment failure could seriously affect the health care of individuals. Counseling patients is required to verify their understanding of medications and appropriate outcomes of drug therapy. This counseling can increase medication compliance and decrease patient morbidity. Decision-making is required in the Drug Utilization Evaluation process. This review performed by the pharmacist results in better patient care and can lead to cost savings. The pharmacist's decisions are also involved in formulating policies and procedures affecting department operations.

Key Essential Functions and Accountabilities of the Job: Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, lab value abnormalities, and follow up via patient chart screening. Provide clinical consultation and clarification and resolve concerns with the prescriber prior to dispensing. Document all clinical activities and interventions accurately. Enter prescriptions into pharmacy software. Verify finished prescription orders in a timely manner. Orders must be filled accurately while maintaining all required documentation. Work independently with minimal supervision. Organize and prioritize work assignments. Ensure pharmacy services are provided efficiently. Advise patients clearly and accurately regarding their medications. Verify patient understanding of medications, including the drug name, strength, usage, and expected changes. Communicate regularly with patients and healthcare providers to resolve problems and provide optimal care. Perform chart reviews for prescription refills and medication therapy management. Write recommendations for providers and request prescription refills on behalf of patients. Provide drug information, drug therapy consultation, and staff education related to drug therapy. Direct pharmacy support personnel. Verify the daily activities of pharmacy technicians. Serve as a role model and educator for pharmacy students and technicians. Participate in clinical and administrative activities, including quality control and quality improvement within the department. Attend meetings and participate on committees as requested. Assure availability, preparation, and control of medications. Select appropriate drugs for formulary and take on additional departmental duties as assigned. Provide health promotion and disease prevention activities related to drug use and preventative drug therapy.

Knowledge, Skills, and Abilities: Must be proficient at reviewing patient medical history, labs, and medication orders using in-depth knowledge of pharmaceutical sciences. Filling prescriptions requires knowledge of state and federal laws governing pharmacy practice. Provide care appropriate to the needs of patients of all ages. Ability to communicate both orally and in writing effectively. Working knowledge of computer application skills is necessary. Math skills are required for accurate calculation of drug dosages and compounding formulations. Ability to work cooperatively within the pharmacy staff and exercise sound professional judgment. Handle frequent interruptions and adapt to changes in workload and work schedule. Ability to work independently, set priorities, make critical decisions, and respond quickly to emergency requests.
