NextCare Providers LLC is looking for dependable and compassionate Licensed Practical Nurses (LPNs) to provide one-hour intermittent skilled nursing visits to patients throughout the Phoenix metropolitan area. Whether you're looking for a manageable part-time caseload or your goal is to eventually build toward full-time, this position allows you to grow your schedule as compatible patient assignments become available. All newly hired nurses begin on a part-time schedule, including nurses whose goal is full-time employment. As you complete onboarding, training, and begin working independently, additional recurring patients can be added to your schedule based on your availability, location, patient needs, and current agency census. Our goal is to build consistent and sustainable patient assignments rather than immediately filling a nurse's schedule with visits that may not be geographically or operationally appropriate. What Are Intermittent Nursing Visits? This position is different from traditional private-duty nursing or an 8- to 12-hour facility shift. Instead, you'll travel to scheduled patients within your assigned service area to complete approximately one-hour skilled nursing visits. During each visit, you'll provide the ordered nursing services according to the patient's plan of care, complete the required documentation, and then continue to your next scheduled patient. Depending on the patient's needs and physician orders, visits may include: Medication administration Insulin administration Blood glucose monitoring Diabetes management Wound care Catheter care Patient assessments Medication management Monitoring and reporting changes in condition Patient and family education Other ordered skilled nursing interventions within the LPN scope of practice How Scheduling Works Our intermittent nursing model is built around recurring patient assignments and continuity of care. Nurses provide their availability, ZIP code, preferred service area, and travel radius. We then work to match available patients with nurses based on those factors and the patient's established schedule. Starting Part-Time All newly hired nurses begin part-time, even when their goal is full-time employment. Why? Patient care depends on consistency. Building a full home health caseload requires matching multiple patients based on their authorized visit schedules, geographic locations, clinical needs, and the nurse's availability. Starting part-time allows us to: Complete your onboarding and patient-specific training properly Introduce you to your initial patient assignments Establish consistent recurring visits Add compatible patients as they become available Build a more practical geographic route Avoid scheduling changes or delays that could negatively affect continuity of patient care We would rather build your caseload appropriately over time than immediately assign patients simply to fill a schedule and later have to move those patients to another nurse. Nurses interested in full-time employment can be prioritized for additional compatible patient assignments as visits become available. For our intermittent nursing model, full-time generally requires approximately 40 visits per week. The time required to build to a full-time caseload will vary based on patient census, geographic area, nurse availability, and compatible patient schedules. Therefore, an immediate 40-visit schedule cannot be guaranteed upon hire. Nurses who prefer to remain part-time may maintain a smaller recurring caseload based on availability and agency needs. Full-Time Weekend Coverage Expectations Continuity of care doesn't stop when the regular workweek ends. Nurses who transition into full-time status are expected to participate in weekend on-call/coverage at least 1-2 weekend per month. This helps provide backup when unexpected call-outs or urgent coverage needs occur and reduces the risk of delayed or missed patient care. Specific on-call and weekend coverage procedures will be reviewed before transitioning into full-time status. Service Areas Patient assignments are available throughout the Phoenix metropolitan area, including: West Valley East Valley Central Phoenix Whenever possible, nurses are matched with patients based on geographic location and preferred travel radius. During the application process, we'll ask for your home ZIP code and how far you're comfortable traveling so we can determine whether current or upcoming patients align with your preferred area. What You'll Do As an intermittent home health LPN, you will: Provide skilled nursing care in patients' homes according to physician orders and the established plan of care. Complete assigned one-hour visits within required scheduling windows. Perform ordered nursing interventions safely and within your LPN scope of practice. Complete accurate and timely documentation for every patient visit. Follow Electronic Visit Verification (EVV) requirements. Review and follow patient-specific care instructions. Monitor patients and appropriately report changes in condition. Educate patients and families regarding medications, treatments, disease management, and safety when applicable. Communicate professionally with patients, families, scheduling, administrative staff, and clinical leadership. Promptly communicate delays, call-outs, scheduling conflicts, or inability to complete an assigned visit. Report patient refusals, medication concerns, incidents, missed visits, changes in condition, and other clinical concerns according to agency procedures. Maintain HIPAA compliance, confidentiality, professionalism, and patient dignity. Participate in required agency training and nursing in-services. Maintain required licenses, certifications, and compliance credentials. Communication & Documentation Matter Because our nurses work independently in the field, communication and documentation are essential parts of this position. We're looking for nurses who don't disappear once they leave a patient's home. Nurses are expected to remain responsive to appropriate clinical and administrative communication and keep the agency informed when something may affect a patient or scheduled visit. Our team currently utilizes: WellSky/Kinnser for clinical documentation Microsoft Teams as a primary agency communication platform Electronic Visit Verification (EVV) Adobe electronic forms and signatures Gusto for HR and onboarding processes Don't have experience with every system? That's okay. Training on agency-specific workflows will be provided during onboarding. Applicants should, however, be comfortable using smartphones, applications, electronic documentation systems, and digital communication tools as part of their everyday work. Article 9 Certification Because NextCare Providers LLC provides nursing services to individuals enrolled with Arizona's Division of Developmental Disabilities (DDD), Article 9 Certification is required before independently working with applicable DDD members. Don't Have Article 9 Yet? You can still apply. Candidates who otherwise meet the position requirements may complete Article 9 as part of their pre-employment requirements before receiving an independent DDD patient assignment. Article 9 training provides education regarding the rights and protections of individuals receiving DDD services and the responsibilities of professionals providing their care. Training can be completed through an approved provider. One option is Arizona Autism United Article 9 Training. Additional information and provider training resources are available through the Arizona Division of Developmental Disabilities. Already certified? Great! Nurses must be able to safely perform the essential functions associated with their assigned patients and communicate when additional assistance or support is required.
