(LPN) Charge Nurse

Job Summary The Charge Nurse is responsible for coordinating and supervising assigned nursing personnel and clinical suites. This position requires performing routine and standardized duties to support competent nursing staff management. The Charge Nurse coordinates the assigned nursing staff according to the philosophy, objectives and policies/procedures of New Horizon Family Health Services, Inc. (NHFHS). Responsibilities include daily duties of direct practitioner support and clinical patient care. Responsibilities Patient Assessment and Care Team Support Collects and documents demographic data and history from the patient, family and others. Identifies gross manifestations of common diseases and abnormal conditions. Identifies gross states of faulty emotional and mental health adjustment. Obtains and documents vital signs and immediately reports abnormal/emergent results to the practitioner. Supports practitioners in the delivery of medical care, productivity and patient flow management. Consults with practitioners and other health care team members to coordinate the provision of care and patient education in conjunction with other services and care plan activities. Supervision Evaluates the assigned nursing staff and completes job performance reviews as required. Collects information and prepares necessary reports and performance documentation. Works with the Practice Manager to maintain appropriate clinical staffing coverage. Orders supplies and other resources as required for execution of nursing tasks. Works with the NCSSD to coordinate Nursing, Medical Assistant and Patient Care Technician Orientation to promote competency and quality in the discharge of nursing duties. Quality Implements NHFHS Nursing / Clinical Department policies and procedures. Works continually to assure delivery of quality, safe, efficient and competent nursing care. Assists in contributing input/info into the nursing personnel budget. Contributes suggestions, ideas and information and works with the NCSSD to refine nursing policies and procedures. Participates in CQI, other internal committees and special activities that promote improvement of patient flow in the clinical area. Community Maintains good public relations throughout the community. Assists, as assigned, with special projects involving community partners and the nursing staff. Qualifications Education LPN with a valid South Carolina Board of Nursing License (Required). Additional certifications as applicable. Experience At least two years of community nursing experience (Required). Minimum of three years’ experience (Preferred). Other Requirements Maintain a current and active Basic Life Support (BLS) certification from an approved provider. Certification must be obtained through a hybrid or in‑person course offered by the American Red Cross or the American Heart Association.
